Home Care Plus Community Healthcare and Hospice is a century old professional home care agency based in Guilford and Hamden, CT. Home Care Plus Community Healthcare and Hospice is a safety net to thousands of older adults, newborns and family caregivers. As part of our non-profit, community based mission, we’ve been providing skilled nursing, therapeutic and preventive health services in the community for more than 100 years. This year alone, Home Care Plus Community Healthcare and Hospice staff will make 310,000 nursing, physical and occupational therapy, social work, and home health aide visits (almost 1,400 visits a day!). We also provide community wellness programs, immunizations, health screenings and support for family caregivers through our Caregiver Support Network.

It is the mission of Home Care Plus Community Healthcare and Hospice to promote and preserve the health, safety, dignity and independence of individuals and families by providing therapeutic and preventive health care services in their place of residence and in the community.

Barbara Katz, RN, MSN, is responsible for all marketing, business development, the Caregiver Support Network and community health promotion. She is a graduate of the Yale School of Nursing. Barbara has been a Family Nurse Practitioner, a health care manager and a health care training consultant.

Barbara was the founder of VNA Community Healthcare’s Caregiver Support Network, a program that provides one on one consultation for family caregivers, an 800 number for advice, a walking group and caregiver newsletters and seminars. Barbara has a particular interest in helping people navigate the health care system. She is the author of two booklets published by the VNA – How to Be a HealthCare Advocate for Yourself and the People You Love and Avoiding an Eldercare Crisis.
