Bernadette DiGiulian, is a Geriatric Care Manager and owns her own company, Elder Caring LLC. Geriatric Care Management is a comprehensive approach to help older adults and their families. Elder Caring LLC offers a full range of services that assist clients in achieving the best quality of life possible, which includes managing challenges such as chronic illness, dementia, physical disabilities and other life changes. In addition, Elder Caring LLC assists in maneuvering a very complex healthcare system. New options or already existing resources are suggested that make caring for an aging person less confusing and stressful.
Elder Caring LLC provides assessments, consultations, care management, help with placement, advocacy and help with home care. Bernadette DiGiulian of Elder Caring LLC is a facilitator, a problem solver, a strategist, a counselor, an advocate, a mediator and a change advocate. She works closely with family members, physicians, attorneys, bank trust officers, financial planners, hospitals, nursing homes and rehabilitation centers, conservators, community agencies, homecare agencies, and other community resource specialists. Bernadette will manage, tweak, change and supervise care as necessary. In many cases, where family members live at a distance or the elderly person has no family, Bernadette becomes somewhat of a surrogate daughter, making sure that care is as good as she would give her own 95 year old Mother, her most important teacher.
Bernadette DiGiulian is a graduate of Yale Divinity School where she concentrated in medical ethics and pastoral counseling. She has worked in the field of geriatrics for twenty-six years. She is a Certified Chaplain and a member of the National Association of Chaplains. Bernadette did her training at Yale New Haven Hospital, Hospital of St. Raphael/New Haven, and Masonic Health Care Center, Wallingford, CT. Bernadette has worked at Branford Hills Health Care Center, for Church Homes Inc., and as Director of Resident Services at Evergreen Woods in North Branford, CT. She has supervised social work, therapeutic recreation, large volunteer groups and directed orientation for all disciplines. She and a nurse colleague started one of the first Assisted Living Programs in the state of CT. That program received a 98% rating from JACO. Bernadette is familiar with a full range of care. Bernadette DiGiulian was a public school teacher in New York, Connecticut and Pennsylvania. She attended the State University of New York at Cortland and Colgate University before she attended divinity school. She has been a resident of Branford, Connecticut for forty years and has been active in many shoreline activities. She is one of the founders of the Shoreline Eldercare Alliance, a group whose primary goal is to raise public awareness about issues and services important to older adults.
